Medium settings with an Intel 4600? Low is pretty much all you could handle, and your GPU is holding you back.

No. It shouldn't be. Most likely your CPU, it's only running 2.2GHZ and that's very very low for a CPU like yours. But Considering it's a laptop, there isn't much more you could really do to help it. I recommend just trying to play on low for the time being.

I'm playing on an Alienware M14X R2 Laptop
Geforce GT 650m (overclocked at +135 and +200)
    6GB RAM
    Intel Core I5-3210M @ 250Mhz
I run the game with resolutions on high, lighting high, everything else medium (ragdolls off), billinear filtering and 2x MSAA AA at around 50-60FPS with occasional 5-10 second drops to around 30 FPS.
All this to say your 750m and I7 processor should be plenty viable to run the game on higher settings.

Okay, here's some suggestions, all fairly simple (In no particular order)
    - ***TRY THIS FIRST***: (If this works, you may not need to do anything else. Except ensuring drivers are installed is important) Ensure you are not using your integrated graphics, but instead your dedicated graphics card
-Try installing Nvidia Inspector (Look up a guide on how to use it)
    - Try overclocking your GPU (With caution, again look up a guide on how to do it. It can potentially be dangerous if you just boost the clocks all the way, do so methodically)
    - Ensure all current drivers are installed (Geforce Experience is great)
    - Get a good cleaning program (Slim Cleaner I recommend) to quickly clean junk files and potentially improve performance
    - Ensure your laptop is in "high performance" mode while playing.

    Hope some of this helps!

PS- So I don't inflate your expectations of performance, I had to turn down my settings yesterday because for some reason I was getting streaks of sub-30FPS, so I turned some settings down.

Yeah you should be able to play with decent settings considering that my buddy's alienware laptop can play this game fine with almost max settings and its an old i7 with 8gigs of ramm and a gtx 480m

Ive done everything youve suggested(Most was already there) besides the overclocking,Would overclocking the CPU also help?

Interesting. On Laptops, overclocking generally has a minimal effect, I'm not even positive it has a notable benefit. Have you tried unparking your CPUs?

And would it have any negative affects?

Well since you are a on a laptop the neg. effect would be that it could overheat very easily.
